REGIONAL STATE HEALTH ASSESSMENT FORUMS

The Governor’s Office of Health Transformation and the Ohio Department of Health invite you to
participate in Ohio’s next State Health Assessment (SHA) by attending a regional community forum.
These forums provide stakeholders from a wide variety of sectors with an opportunity to identify priorities, strengths, challenges and emerging trends for all areas of the state.

Join us for an interactive forum where
you can provide input on the health of your region and state. This event is open to all stakeholders who are interested in improving the health and quality of life in Ohio.
